29 CFR 1910.28(b)(6)(i): The employer did not ensure each employee less than 4 feet (1.2 m) above dangerous equipment was protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system when the equipment was not covered or guarded to eliminate the hazard: (a) Paint Department, Washer - On or about March 2, 2017 walkway over tanks containing hazardous chemicals including hydrochloric acid was open on both sides.
