29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):(a). On or about February 22, 2017, a baler machine operator was exposed to machine hazards associated with the unexpected energization of moving parts while servicing the bailing wire inside the bale chamber of a Machinex baler machine. The employer did not ensure the baler machine operator implemented hazardous energy control procedures by turning off the main disconnect switch on the machine control panel [per the 1910.147(d)(2) requirements], isolating the energy sources by operating the master control relay (A key) [(d)(3)], affixing a lockout device (lock) on the main control disconnect switch [(d)(4)(i)], and verifying the baler was de-energized [(d)(6)].
