29 CFR 1926.502(d)(15)(I): Anchorages used for attachment of personal fall arrest equipment were not designed, installed, and used as a part of a complete personal fall arrest system which maintains a safety factor of at least two: a) West side of roof, near center: On or about February 9, 2017, employees were exposed to a fall of greater than 6 feet from the ground level below, while being attached to improperly installed hinged roof anchors during the removal of metal sheeting and corrugated fiberglass skylights. b) East side of roof, near ridge line: On or about February 9, 2017, employees were exposed to a fall of greater than 6 feet from the ground level below, while being attached to improperly installed hinged roof anchors during the removal of metal sheeting and corrugated fiberglass skylights.
