29 CFR 1926.452(j)(1): 29 CFR 1926.452(j)(1): Pump jack brackets, braces, and accessories were not fabricated from metal plates and angles. Each pump jack bracket did not have two positive gripping mechanisms to prevent any failure or slippage. (a) The employer did not ensure that the two positive gripping mechanisms on the Hydro-Mobile scaffold were working properly to prevent failure or slippage. This condition exposed employees to a collapse of the scaffold and a 40 foot fall hazard.
