29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached: (a) on the jobsite - employees were exposed to a 9ft,2in fall hazard, in that, three to four employees were attached to one High Visible Rock Solid (model CRPK-anchor) butterfly anchor rated for 5,000 pounds located at the peak of the roof of the main sanctuary building (6:12 slope); observed on or about 2/2/2017.
