29 CFR 1915.12(d)(1): The employer did not ensure that each employee entering a confined or enclosed space or other areas with dangerous atmospheres was trained to perform all required duties safely. Location: Naval Base Coronado Port Operations in the boatyard: Workers were not provided training on recognizing and eliminating potentially hazardous atmospheres while performing cold work on the vessel adjacent to the fuel tank. Employees were exposed to fire and/or explosion hazards while using spark-producing equipment in the presence of flammable vapors to torque the generator's foundation bolts to the vessel.
