29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a.) On or about January 25, 2017, setup employees were exposed to crushing and struck by injuries in that the (30-0021) Fanuc Robot and the #73 Cincinnati Milacron plastic injection molding machine were not locked out prior to adjusting a gripper on the robot inside the press. The employer did not ensure that energy control application steps to control the electrical energy source were implemented prior to the maintenance tasks, the employer did not: · Physically operate energy isolating devices to control energy sources and affix lockout device(s) · Verify that isolation and deenergization of the machine or equipment had been accomplished
