29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) Wyvern Site, Track Location OOC 418.1 - On or about January 24, 2017, the employer exposed employees to an engulfment hazard, in that employees working in an unprotected trench, ranging from 6 to 9 feet deep with walls that had been cut almost vertically, were not provided with a safe means of access and egress such as a ramp or ladder.
