29 CFR 1910.140(d)(2)(ii): The employer did not ensure that personal fall arrest systems were rigged in such a manner that the employee cannot free fall more than 6 feet (1.8 m) or contact a lower level. On or about January 29, 2017, on the rooftop of a four story building where employees were exposed to a seventy-foot fall hazard when cleaning windows using a Sky Genie rope descent system without a personal fall arrest system rigged to prevent a free fall more than six feet or contact a lower level.
