29 CFR 1910.1030(d)(2)(i): Engineering and work practice controls were not used to eliminate or minimize employee exposure: a) Operating room, on or about March 17, 2017 - Surgical technicians handled scalpel blades that were not equipped with blade covering protectors used during orthopedic and vascular surgical cases. b) Surgi-Center, on or about March 17, 2017 - Surgical technicians handled scalpel blades that were not equipped with blade covering protectors used during orthopedic and vascular surgical cases. c) Interventional Radiology, on or about March 17, 2017 - Residents used Hoover needles that were not equipped with safe sharps protective devices. d) Operating room, on or about March 17, 2017 - Residents used Hollow point needles that were not equipped with safe sharps protective devices. e) Nursing units, on or about March 17, 2017 - RN's used U500 Insulin syringes that were not equipped with safe sharps protective devices for insulin injections. f) Histology, on or about March 17, 2017 - Employees removed scalpel blades that were not equipped with blade covering protectors from reusable holders while preparing specimens.
