29 CFR 1910.178(q)(6): Powered industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer, by the addition of extra parts not provided by the manufacturer, or by the elimination of any parts without the manufacturer's approval: a) Plant - On or about 01/11/17 and at times prior thereto employees operated the Yale #9 (LPG-powered) forklift, the Yale #2 (Electric) forklift, the Yale #3 (Electric) forklift, the Yale #4 (Electric) forklift, and the Yale #6 (Electric) forklift. All of them were modified with the removal of the original front rack, exposing the roller chains and the hydraulic hoses to damage caused by the displacement of the load on the forks. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
