29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: (a) 1511 Washington Street East: On or about January 10, 2017 and times prior, an employee was working in a trench on his knees installing underground fire service to an apartment building using a TapMate pipe drilling machine to drill into a 4in. cast iron underground water main that contained between 80 psi -113 psi without the use of safety glasses, goggles, and/or a face shield.
