29 CFR 1910.184(c)(13): A sling was loaded in excess of its recommended safe working load as prescribed by the sling manufacturer: a) Jobsite, Bay One, 1889 Mayview Road, Bridgeville, PA 15017: On or about August 26, 2016, an alloy chain sling (3/8", X 14'1", type TOG, GR80) was used to lift loads in excess of its recommended safe working load. The alloy chain sling was used to lift the back end of a Hoist brand powered industrial truck (model number FKS15X, serial number 21347) that weighed approximately 37,000 pounds. The alloy chain sling would have been overloaded in any configuration listed on the certificate of test sheet provided by the sling manufacturer. Employees were exposed to crushed-by and struck-by hazards while using the alloy chain sling to lift the back end of the Hoist truck. b) Jobsite, Bay One, 1889 Mayview Road, Bridgeville, PA 15017: On or about January 6, 2017, an alloy chain sling (3/8", X 14'1", type TOG, GR80) was used to lift loads in excess of its recommended safe working load. The alloy chain sling was used to lift the back end of a Hyster brand powered industrial truck (model number H155FT, serial number K006V02985N) that weighed approximately 21,580 pounds. The back end of the truck was lifted with the alloy chain sling using a single leg configuration. This configuration would have provided a working load limit of approximately 7,100 pounds. Employees were exposed to crushed-by and struck-by hazards while using the alloy chain sling to lift the back end of the Hyster H155FT truck. c) Jobsite, Bay One, 1889 Mayview Road, Bridgeville, PA 15017: On or about January 4, 2017, an alloy chain sling (3/8", X 14'1", type TOG, GR80) was used to lift loads in excess of its recommended safe working load. The alloy chain sling was used to lift a Hyster brand powered industrial truck (model number HJ120FT, model number S005V01763L) that weighed approximately 17810 pounds. The back end of the truck was lifted with the alloy chain sling using a single leg configuration. This configuration would have provided a working load limit of approximately 7,100 pounds. Employees were exposed to crushed-by and struck-by hazards while using the alloy chain sling to lift the back end of the Hyster HJ120FT truck.
