29 CFR 1926.1203(a): Before it began work at a worksite, the employer did not ensure that a competent person identified all confined spaces in which one or more of the employees it directed may have worked, and/or did not identify each space that was a permit space, through consideration and evaluation of the elements of that space, including testing as necessary: The employer is failing to protect employees from hazardous atmospheres associated with confined space entry. This was most recently documented on or about December 15, 2016 at the jobsite located at or near 507 Coleman Road in Belton, Missouri. Prior to work beginning, the employer did not provide or require an evaluation of the hazards associated with entering the sewer access manhole by a competent person.
