29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed: a) Throughout Facility - On or about February 9, 2017 and times prior to, the employer had not conducted an inspection of the energy control procedures on an annual or more frequent basis. In addition, the employer did not ensure that all authorized employees were identified; all authorized employees were properly following the energy control procedures; or that adequate group lockout/tagout procedures had been implemented, thus exposing employees to caught-in, pinch point and crushed-by hazards.
