Text For Citation: 01 Item/Group: 002 Hazard:
29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. a) On or about 12/01/2016, in the Stamping Department; Lockout procedures were not utilized when employees were fixing a jam at the HEIM 200 ton press, exposing employees to struck-by hazards. b) On or about 12/01/2016, in the Stamping Department; Lockout procedures were not utilized when employees were removing/replacing dies which would protect the press operators from the downward motion of the ram of the mechanical power press such as, but not limited to, HEIM 200, Bliss C-22,C45,C60 & C75. Employees were exposed to crushing/amputation hazards. ABATEMENT DOCUMENTATION REQUIRED
