29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: a. On or about December 13, 2016, in the filter press area, an employee was exposed to fall hazards when working on a platform fifty nine (59') inches above the ground which was not provided with standard guardrails. The platform was also located next to dangerous equipment.
