29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed: The employer is failing to protect employees from caught between, amputation, fall, electrical and struck-by hazards associated with not having performed and documented the job hazard assessment process. This most recent occurrence of this was evident during the inspection of the facility located at 13800 S. 54th Street, Roca, NE; where the employer has maintenance employees replacing/repairing the tarp control arm assembly on side dumper trailers while exposed to a fall greater than four feet without fall protection PPE.
