29 CFR 1926.106(d): A lifesaving skiff was not immediately available at locations where employees were working over or adjacent to water: a) Bridge - On or about November 4, 2016, employees were working over and near an 11' deep canal from a suspended scaffold. A lifesaving skiff was on site but not immediately available, it was on the opposite side of the canal as the work was taking place. An employee ran over 400' to reach the skiff. -On November 4, 2016 an employee drowned in the canal, after falling from the suspended scaffold partially collapsed.
