29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: (a) On or about 10/31/2016 - at 1305 River Rd, Helena, AL, when excavating Colonial Pipeline Company's Line 1 containing A4 gasoline, the exact location of the pipeline was not determined by safe and acceptable means, such as but not limited to, Colonial Pipeline Company's Excavation and Backfill Procedure MP-01-001 Rev 5 that requires the pipeline to be clearly marked in the planned excavation area using both a line locator and a probe rod; confirming the pipeline depth and horizontal location in at least two places in the planned excavation work area using two of the following methods: line locator, hand excavation, probe rod, or soft dig technology; and ensuring mechanical excavation is done parallel to pipeline.
