29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. The employer is failing to protect employees from crushed-by and struck-by hazards. This was most recently documented on October 17, 2016, when employees were not provided with machine specific procedures for the isolation and control of hazardous energy. The employer had not developed or implemented machine specific procedures for locking out production equipment including but not limited to the Weber Bag Machine Model #1667 Serial #744, known by the company as Machine 9.
