Text For Citation: 01 Item/Group: 002 Hazard:

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: The employer is failing to protect employees from potential crushing hazards associated with working in the vicinity of equipment used on the jobsite. This was most recently documented on October 17, 2016 at a worksite located at 7723 Heritage Highway, Jefferson City, Missouri where employees had not been properly trained on the hazards of working in the vicinity of concrete pumping trucks. Some of the hazards associated with this machine include, but are not limited to, bursting couplings, pipes, tipping hazards, boom collapse, and pinch point hazards. An employee was killed when he struck and became trapped in the frame-mounted outrigger was being retracted in to the side of the machine.
