29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and regulatory requirements for controlling and eliminating hazards in the workplace: a) Employees were exposed to electric shock/ burns. Employees were not instructed on the means, methods and pre-task review to work on energized panels (480VAC) when connecting wiring in panels DPH-40B-2 and 40-B-4. An arc flash occurred when the employee was working in the panel. Location: 388 Greenwich St, New York, N.Y., 40th fl mechanical rm. On or about 10/08/2016 NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
