29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was not isolated from the energy source and rendered inoperative: At 355 Paul Avenue in St. Louis, MO 63135 (Fabrication Areas) - On or about 12/06/2016, employees were exposed to the hazard of being contacted by hazardous energy while performing maintenance on equipment, such as, but not limited to, an "Peerless" (Model: 1218MHA; Serial No. MH68-357-A-65) metal cutting band saw, and the employer failed to establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was not isolated from the energy source and rendered inoperative.
