29 CFR 1910.333(b)(2)(i): The employer did not make a written copy of the procedures outlined in paragraph (b)(2) of 29 CFR 1910.333 available for inspection by employees and by the Assistant Secretary of Labor and his or her authorized representatives: (a) On and before September 2, 2016, the employer had not developed written Lock Out Tag Out procedures for employees engaged in maintenance work tasks involving an Energized 480 Volt Electrical Panel, located in the Electrical Vault of the Mill Basemen. As a result, an Arc Flash occurred seriously injuring the employee. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
