Text For Citation: 01 Item/Group: 002 Hazard:
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees are exposed to struck by hazards: The employer is failing to protect the employees from struck by hazards associated with lifting and setting steel beams in support of steel erection. The most recent occurrence of this was found on the job site located at 3701 South 10th Street in Omaha, Nebraska where the employer was using an Extreme model XR1255 all-terrain forklift with a model 1360B HD JIB attachment and an All Material Handling model BCD-05 beam clamp as means of lifting and setting the intermediate beams on a new commercial building construction when the beam slipped through the beam clamp and struck an employee. FEASIBLE AND ACCEPTABLE METHODS OF ABATEMENT TO CORRECT THIS HAZARD INCLUDES BUT IS NOT LIMITED TO: a. The employer must utilize proper rigging equipment to lift the intermediate beams into place. b. The employer must follow the manufacturers safety information to include but not limited to: i. Do not use for lifting, supporting, or trans-porting people or lifting supporting loads over people. ii. To avoid an unbalanced load which may damage the trolley, the hoist must be centered UNDER the trolley. iii. Trolley wheels ride on the top of the lower flange of the beam. c. Prior to lifting any materials into place the employer must evaluate the lift site, identify the hazards, develop a lift plan that better supports keeping the area around the lift clear of employees and safe to operate.
