29 CFR 1910.119(d)(3)(i)(B): Information pertaining to the equipment in the process did not include piping and instrument diagrams (P&ID's): a) Establishment - Building 3, alkyd resin process The employer did not ensure that the written process safety information included all the necessary information pertaining to the equipment in the process when basic data such as, but not limited to, vessel size, piping material specifications, equipment numbers, rupture disk burst pressure and failure modes of automatic valves were not included in PID 13-82-H-23. b) Establishment - Building 3, alkyd reactor AR-1 The employer did not ensure that written process safety information accurately reflected existing equipment in the process in that PID 13-82-H-23 did not show a manual valve on top of the condenser for AR-1, a gate valve on the AR-1 line to its decanter and a temperature gauge on the cooling water supply line to AR-1. c) Establishment - Building 3, alkyd reactor AR-1 The employer did not ensure that written process safety information accurately reflected existing equipment in the process in that PID 13-82-H-23 showed 1) a 4 inch line at the top of a condenser reduced to 3 inches, a tee and reducer to a 2 inch line that is connected by flexible tubing to the header to the scrubber rather than smaller diameter piping hard piped to the header and 2) a 12 inch line from the reactor to the condenser rather than an 8 inch line. Violations occurred on or about 6/29/16.
