29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: a) On or about 08/01/2016 - Maintenance department, Battenfield injection molding machine, TR-US 02, employees performing maintenance on a DFE Valve with multiple sources of energy including 480 VAC, pneumatic pressure, and gravity were not provided adequate devices to ensure that he machinery could not be reenergized while employees performed maintenance. b) On or about 08/01/2016 - Maintenance department, employees performing maintenance on machinery such as, but not limited to, the potting machine were not provided the adequate amount of devices to block, lock, or tagout the machinery operating on electrical, pneumatic, and gravitational sources of energy. c) On or about 08/01/2016 - Maintenance department, an employee performing maintenance on machinery such as, but not limited to, a drill machine was not provided the adequate amount of devices to lock and tagout the machine operating on electrical power and pneumatic pressure.
