Text For Citation: 01 Item/Group: 002 Hazard: UNAPEQUIP

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employee(s) in that employee(s) were exposed to the hazards of a failure within the hoist or other components during loading/unloading operations or while riding the freight platform. Establishment: Employees involved with loading/unloading the freight platform were exposed to serious injury as a result of the following: 1. The freight platform was not equipped with an arresting device that would function in the event of a failure with the hoist or any other component. 2. Rated load capacity was not conspicuously posted on the freight platform. A feasible and useful method to correct these hazards, among other methods, is to replace the existing freight platform with a unit that complies with the Massachusetts State Elevator Code and to conspicuously post the rated load capacity of the freight platform.
