Executive Order 12196 Section 1-201(a) of the Occupational Safety and Health Programs for Federal Employees: The Head of the Agency did not furnished employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious harm to employees in that employees were exposed to the hazard of being caught in a moving conveyor: a) Along PSM-1 and PSM-2 the emergency stop cord is on the wrong side of the aisle-way so as to be ineffective in an emergency situation. b) Emergency conveyor stop cords are needed along the tray line around the bull wheel from grid E-2 thru H-2; along PSM-3 and PSM-4 tray lines. c) An emergency stop cord is needed along the PSM-2 south bull wheel tray line. d) Emergency stops have not been installed along lengths of the PSM-1, PSM-2, PSM-4, SSM-1 and SSM-2 sorter machines.
