Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards from falling loads: a) Leitner-Poma of America, Inc. & 2510 Foresight Cr., Grand Junction, CO: On or before 6/18/02, the employer did not ensure that shop-made under-the-hook lifting devices were tested and rated in accordance with the requirements of ASME B30.20-1985, The American Society of Mechanical Engineers - An American National Standard - Safety Standard for Cableways, Cranes, Derricks, Hoists, Hooks, Jacks, and Slings - Below-the-Hook Lifting Devices. These deficiencies are as follows: 1. Job-made lifting devices such as the hanger tube lifting clamp in the Machine Shop and the sheave train lifting device in the Sheave Area were not marked as to their load rating, manufacturer, lifter weight, and serial numbers which are required by paragraphs 20-1.2.1 (a) and (b) of ASME B30.20-1985. 2. The lifting devices had not had an initial rated load test, using the attachment methods in place, as required by paragraph 20-1.4.2(a) of ASME B30.20-1985. Abatement Note: One feasible abatement method is to test and rate the hoists and lifting devices in accordance with ASME B30.20-1985.
