29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. On 6/15/16, at the Athens facility, employees were not utilizing Lockout procedures for various service and maintenance activities on site including: a - On 6/15/16 a sanitation employee received an amputation while attempting to remove paws from a moving shackle line. b - Sanitation employees did not utilize the documented LOTO procedure for locking out shackle lines for service and maintenance activities.
