29 CFR 1926.405(b)(2): All pull boxes, junction boxes, and fittings were not provided with covers: (a) On or about June 13, 2016, and at times prior thereto, subcontractor employees were exposed to electrical hazards within 13 inches of an exposed, live temporary lighting electrical box, while accessing steel conduit from a rack, where no cover plate was installed on the face of the electrical box. NOTE: Abatement certification is required for this item.
