29 CFR 1910.178(q)(1): Power-operated industrial truck that were not in safe operating condition and were not removed from service. Instance # 1: (a) Date of Violation: On or about 06-14-2016 and times prior thereto, located at U.S. Postal Service Merrifield Processing and Distribution Center, 8409 Lee Hwy. Merrifield, VA 22081, on the West Loading Dock. Condition: Employee(s) operated powered industrial equipment, a powered pallet jack, with improper electrical repairs to the forklifts onboard 110 volt charging system cord. The cord had been replaced with what appeared to be a section of extension cord and wrapped with USPS Parcel packaging tape to secure the splice. Instance # 2: a) Date of Violation: On or about 06-14-2016 and times prior thereto, located at U.S. Postal Service Merrifield Processing and Distribution Center, 8409 Lee Hwy. Merrifield, VA 22081, on the West Loading Dock. Condition: Employee(s) operated powered industrial equipment, a powered pallet jack #34, with half of the entire steering and control handle broken off and missing. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
