29 CFR 1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146 (permit required confined spaces) acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under 29 CFR 1910.146: a.) Terminal Ready Mix, Inc. located at 524 Colorado Ave. Lorain, OH; On or about May 8, 2016, the employer did not ensure each employee required to enter a permit required confined space was trained with the knowledge and skills necessary to safely perform entry of a permit required confined space. Employees were not trained in required permit documentation measures, that atmospheric testing was needed when entering and while in the cement mixer, and the need to have an attendant outside the pace to help the entrant in an emergency situation. Employees are exposed to hazards from the rotating mixer and hazards associated with welding fumes.
