29 CFR 1926.453(a)(2): Field modification of the aerial lift was not certified in writing by the manufacturer or by any other equivalent entity, such as a nationally recognized testing laboratory, to be in conformity with all applicable provisions of ANSI: (a). Worksite located at 301-311 N. Liberty Drive, Independence, Missouri- Employees were exposed to fall or collapse hazards in that the employer field modified the lift to perform actions the lift was not rated or tested to perform.
