29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: (a) Hanna Electrical Contracting, Inc. at 1833 E. Mulberry St., Fort Collins, CO 80524: On and before June 6, 2016, the employer did not ensure that employees were protected from falling through skylights by means of personal fall arrest systems, covers, or guardrail systems erected around skylights, in that an employee who accessed and worked from the flat roof of a commercial building during the removal and installation of exterior building lighting fell through a skylight and landed approximately 24 feet below. This condition resulted in a fatality.
