HWY 332 in Lake Jackson, TX on or about April 11, 2016; employees worked within a 14 foot deep excavation and safe means of egress was not provided as to require no more than 25 feet of lateral travel, exposing employees to caught-in and engulfment hazards. b) At 4815 Fairmont Pkwy in Pasadena, TX on or about July 11, 2016; employees worked within a 14.5 foot deep excavation and safe means of egress was not provided as to require no more than 25 feet of lateral travel, exposing employees to caught-in and engulfment hazards.
