29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). (a) Facility, 204 Franklin Valley Road, Dalton, PA - A Combilift Multidirectional Sideloader Model CL300 (Serial No. 10778) and a Crown Forklift Model SC4520-35 (Serial No. 9A161057) were used to transport metal stock and completed product by operators that had never received equipment specific formal instruction, practical training, or an evaluation as to their ability to safely operate the equipment, exposing employees to struck-by and crushed-by hazards associated with powered industrial truck operations, as observed on or about May 16, 2016.
