29 CFR 1926.959(d)(1): Minimum approach distance. Mechanical equipment was not operated so that the minimum approach distances, established by the employer under � 1926.960(c)(1)(i), was maintained from exposed energized lines and equipment: At Freymuth Road near Eisenbath Road intersection in Saint Paul, MO 63366 - On or about 05/05/2016, employees were exposed to the hazard of being contacted by electricity, approximately 7,200 volts, where mechanical equipment, such as, but not limited to, an "ALTEC" bucket truck (VIN No. 544342), was not operated so that the minimum approach distances, established by the employer under � 1926.960(c)(1)(i), was maintained from exposed energized lines and equipment. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19
