OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught in, rotating parts or amputation hazards. a) On or about May 11, 2016, the employer did not ensure that the Cincinnati Press Brake was equipped with operational emergency stop controls exposing employees to being caught-in and amputation hazards. b) On or about July 12, 2016, the employer did not ensure that the Lidder Machine was equipped with operational emergency stop controls exposing employees to being caught-in and amputation hazards. c) On or about July 12, 2016, the employer did not ensure that the Pneumatic Press Machine was equipped with operational emergency stop controls exposing employees to being caught-in and amputation hazards. d) On or about May 31, 2016, the employer did not ensure that the Rando Feeder was equipped with sufficient amount of operational emergency stop controls exposing employees to being caught-in and amputation hazards. e) On or about July 7, 2016, the employer did not ensure that the Piranha was equipped with operational emergency stop controls exposing employees to being caught-in and amputation hazards. f) On or about May 31, 2016, in Building 115, Meltog Machine, employees were exposed to rotating parts, caught-in hazards and/or possible amputation hazards due to unexpected energization of the unguarded foot pedal on the Meltog #1 machine.
