29 CFR 1910.23(b)(1)(i): Every wall opening from which there was a drop of more than 4 feet was not guarded by one of the following: Rail, roller, picket fence, half door, or equivalent barrier. Supply New England, 582 Quaker Highway, Uxbridge, Massachusetts; On May 4, 2016 employees were working from the mezzanine and upper levels of the warehouse and were exposed to falls of approximately 10 feet to the ground below due to the lack of standard railing. a. Fall from the mezzanine by the loading docks - approximately 8 feet 6 inches b. Fall from the mezzanine in the back by the battery charger - approximately 13 feet 10 inches
