This condition was last observed on and about 05/05/16, at 16051 MO-HWY 47, Marthasville, MO 63357. Inside of Building #1, specifically the lower level, a specific lockout/tagout procedure is not developed or implemented for the Strippit LVD Turbo 150T Press Brake, m/n PPEB 150/10, s/n 30980. This press brake utilizes electricity, hydraulics, and gravity to operate.
