29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: a) 699 Route 49 East, Teterboro, NJ Two employees were observed in roofing tasks adjacent to the edge of an overhang-type flat roof, and were exposed to an approximate 11' fall, on or about April 25, 2016. An attachment clip used by an employee was stuck open due to metal corrosion, and a fall arrest rope was in poor condition, and red strands were visible. There is no abatement certification or documentation for this item.
