29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees from hazards created by rotating parts: a) For employees in plant 1 located at 421 Leonard Boulevard West, Lehigh Acres, FL, in the mixer 1, Wiggert & Co, Model- HPGM-1500 / S/N 15422, the safety limit switches were not operational and did not maintain the mixer in a stopped position when the access doors were opened while the mixer was energized, on or about 4/13/2016.
