29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting respirator was fit tested prior to initial use of the respirator, whenever a different respirator facepiece was used, and at least annually thereafter: a) USS Forrest Sherman, BAE Systems: On or about 04/06/2016, an employee engaged in abrasive blasting operations wore a type "C" MSA Abrasive-Blast tight fitting full face respirator with hood and was not provided an initial fitted test as a result employees were potential exposed to possible inhalation hazards. b) USS Forrest Sherman, BAE Systems: On or about 04/06/2016, two (2) employees engaged in abrasive blasting operations wore a type "C" MSA Abrasive-Blast tight fitting full face respirator with hood and was not provided with an annual fit tested as a result employees were potential exposed to possible inhalation hazards. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
