29 CFR 1910.146(k)(3)(ii): An authorized entrant entered a permit space and did not have the other end of the retrieval line attached to a mechanical device or fixed point outside the permit space in such a manner that rescue could begin as soon as the rescuer becomes aware that rescue is necessary: (a) On or about March 23, 2016, and at times prior to that date, employees were exposed to physical and health hazards created by the employer failing to ensure that a rescue line was attached to the entrant, requiring another employee to enter the permit required confined space to attach the rescue line to the employee in the event an emergency extraction was needed. Additionally, employees were exposed to physical hazards created by the rescue equipment available for use consisting of an electric winch without a manual back-up or a means of adjusting the force and speed of operation.
