29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) Yard Area: On or about March 9, 2016, and times prior, an employee was exposed to a fall of 9ft. 4in. from the roof of the storage shed while adjusting the security cameras without the use of fall protection. (b) Chipper/Mulcher: On or about March 9, 2016, and times prior employees were exposed to falls of up to 10 feet to the ground while climbing the mulch/chipper conveyor to fill the motor at the top with oil without the use of fall protection.
