29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) BIRDSBORO KOSHER FARMS FACILITY - Employees were performing preventative maintenance tasks such as adjusting and lubricating machines and corrective maintenance tasks such as clearing jams and repairing broken parts on machinery located in the production facility without affixing lock-out or tag-out devices to energy isolation devices, on or about March 2, 2016. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
