29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk was not guarded: a) Water Treatment area; On or about March 29, 2016 and at times prior thereto, the employer exposed employees to slip/trip or caught in hazards in that employees were allowed to walk and work around an 8 inch x 9 inch floor hole that was not protected by a cover or railing.
